I hereby grant permission and consent to my child/children to participate in the Vacation Bibe School Program at Sumter Baptist Temple. I understand that the Vacation Bibe School Program involves one week of indoor and outdoor activities and lessons. In addition to learning about Jesus Christ and His gospel, children will participate in games, crafts, snacks, and other physical activities. Classes will include other children and will be facilitated and led by adult and/or teen volunteers. I acknowledge that such activates carry inherent risks, including the possibility of accidental personal injury or illness. I assume responsibility for all risks of injury, sickness, death, damage, and expenses that may result from my child/children's participation in VBS activities. I release Sumter Baptist Temple, its directors, employees, volunteers, and agents from any and all liability for claims or demands related to injuries, damages, or other losses incurred by the child during the VBS program.
I hereby grant permission for Sumter Baptist Temple to record sounds, images, or video of my child/children while attending Emerald Crossing VBS. I also give my permission for Sumter Baptist Temple, at its sole discretion, to use these sounds, images, or videos in publications (including print, websites, and social media platforms) owned by Sumter Baptist Temple in relation to Emerald Crossing VBS.
